It is my sincere regret that I will be closing my medical practice and retiring permanently on October 21, 2025. I understand this may cause inconvenience, but we will ensure a smooth transition for all our patients. I encourage you to find another healthcare provider as soon as possible. You can contact your health insurance company or the local hospitals that have primary care practices. If you have any emergency or urgency for any reason, please present yourself to the nearest emergency room. 

 

Please request all your medical records by completing the "Medical Record Request" form to the right. We will be emailing you a copy of your final records in a HIPAA-compliant secure format. Please note that once your records have been released to you, your patient portal will be inactivated.
